Truax Patient Services respects that the privacy of your personal information is important to you. Use of Our Web Site. We may collect information about your activities and actions on Truax Patient Services web site in order to personalize future web site content or other communications from Truax Patient Services. You can visit much of our web site without providing us with any personal information. However, some areas of the web sites may require that you register in order to access specific content, features, interactive tools, or services or participate in a particular activity, such as subscribing to a newsletter or joining a program. You may always choose not to provide the requested personal information but you may then not be able to participate in the particular activity.

Responding to Questions or Surveys. On occasion we may request that you provide us with additional demographic or other health information through responses to questions or surveys, or through use of search functions or other forms of feedback that we may use to serve you better.

Information collected from public or third party information sources. We may collect personal information from public or third party information sources to verify information (e.g. professional credentials and identity of a health care professional or validity of credit card information). We may also augment our existing user databases with information from third parties. Some of this information may be personal information, such as change of postal address information.

How Does Truax Patient Services Use My Personal Information?

Your personal information may include the following uses:

  • fulfill a request from you to receive additional information about our services;
  • send health-related materials to you;
  • develop products, services, and programs that you may find useful (e.g., refill reminder, dosing alarm);
  • locate a pharmacy;
  • provide access to your account information; and/or
  • contact you for other purposes ; and/or
  • provide 3rd party insurance carriers for copayment information and/or benefit investigation.

If you do provide personal information to us, you agree that we may contact you through a variety of methods including postal mail, e-mail and telephone in accordance and as permitted by state and federal laws.

Does Truax Patient Services Share Personal Information with Third Parties?

Truax Patient Services may occasionally transfer or share your personal information with third parties who perform certain services on our behalf. Such third parties have agreed that they will only use personal information on behalf of Truax Patient Services.  In addition, Truax Patient Services reserves the right to share your personal information in order to respond to authorized information requests of government authorities, the courts, law enforcement agencies or where required by law, for corporate audits, or to respond to complaints or security threats. If you contact us to report on your experience, or the experiences of your patient, with the use of our medicines, we may contact you for follow-up information. We may also use the information you provide to submit reports of side effects to government agencies such as the U.S. Food and Drug Administration, or as otherwise required by law or governmental audit.

Information that we transfer or share will be used and disclosed in a manner consistent with the terms of this privacy notice. Except as specifically provided in this privacy notice, Truax Patient Services will not transfer, sell or rent your personal information to anyone without your consent.

What Types of Non-Personally-Identifiable Information Does Truax Patient Services Collect and How Is It Used?

We may provide information from our web sites in aggregate form without personally identifiable information to third parties. Any third party that receives aggregated data must agree not to attempt to re-identify the persons to whom it belongs.

Your personal information will be accessible to Truax Patient Services, and to those that use personal information solely for and at the direction of Truax Patient Services. You should also recognize that your use of the Internet and our websites and mobile websites, applications, and online resources is solely at your own risk.

How Does Truax Patient Services Protect the Privacy of Minors?

We do not knowingly or directly collect any personal information from anyone under the age of 18, nor do we knowingly market or direct our web site to anyone under the age of 18.

Can I Choose Not to have Truax Patient Services Collect, Use or Disclose Information About Me?

Yes, it is your choice to provide or not to provide personal information to Truax Patient Services. You may choose not to enter any personal information into a form or data field on our Web sites or applications, and by not using any personalized services provided by our online resources. However, some of our online resources are available only to individuals who provide personal information or who use personalized services.

Furthermore, some of our online resources may request your permission to use and disclose personal information about you in order to add you to our contact lists, to enable you to list yourself in a directory or a social media resource, and to identify and offer additional services and promotions that we believe you might find interesting. You can limit the use of personal information about you by checking or un-checking options provided at the time that you enter that information or that we may offer to you in the future. Some web browsers may transmit “do-not-track” signals to the websites with which the user communicates. Because of differences in how web browsers incorporate and activate this feature, it is not always clear whether users intend for these signals to be transmitted, or whether they even are aware of them. Because there currently is no industry standard concerning what, if anything, websites should do when they receive such signals, we currently do not take action in response to these signals. If and when a final standard is established and accepted, we will reassess how to respond to these signals. In some cases, third parties may be able to collect information about a user’s online activities over time and across different websites when he or she uses our Services.

How Long Will Truax Patient Services Retain My Personal Information?

We will store personal information for as long as the information is required to fulfill business objectives or the purposes for which the information was collected, or until you request that we delete that information, or for as long as may be required by law.
